Payment dispute operations

Payment disputes.
A clearer path to resolution.

Recovant is being built to help health plans and providers manage out-of-network payment disputes—from open negotiation through Federal Independent Dispute Resolution (IDR) under the No Surprises Act (NSA).

One foundation for the work.
A distinct workflow for your role.

NSA / Federal IDREvery stage has a clock.
  1. Initial payment or denial

    30 business days

Start open negotiation within the 30-business-day period beginning when the provider receives the initial payment or denial.

  2. Open negotiation

    30 business days

The negotiation period starts when the open negotiation notice is sent.

  3. Federal IDR initiation

    4 business days

Initiate within 4 business days after open negotiation ends, if unresolved and eligible. Entity selection and offers follow.

  4. Payment determination

    30 business days

The certified IDR entity selects an offer within 30 business days after its final selection.

  5. Payment follow-through

    30 calendar days

Any amount due must be paid within 30 calendar days after the determination.

Agreement reached? Proceed to payment follow-through under the settlement’s terms.

Illustrative playback is compressed and not to scale. Each window has its own starting event; these durations do not add up to a case completion date. Extensions, eligibility and process changes may affect a dispute.

After IDR initiation, entity selection and offer deadlines also apply. Parties have 3 business days after initiation to agree on an entity. Offers are generally due within 10 business days after final entity selection; the determination clock runs from that same selection event. Parties may settle before a determination; settlement payment terms differ from the determination deadline shown.

The 2026 operational changes have phased applicability. This illustration uses the standard windows in the current CMS guidance; it is not a case deadline calculator.
